You've been told before ... Ask someone who is familiar with this form. Either a lawyer, or somebody at the FL agency in charge of it.
on the financial release form from Florida Access it wants me to put a valid from, start date and a end date. I don't know what to put down. I have tried 3 time & they have rejected all 3 times. (different years & different Days.)?
