From my experience, it varies by state.
Usually two witnesses are required.
Notary may or may not be required.
You can go on some of the will sites and view the different pages of a typical will for your state.
I don't know of any way to copy and they have "SAMPLE" written across the page but you should be able to see how many witnesses they show for your state as well as other signature lines.
