For accident reports, questions would range from where is the location of the incident, what is the cars involved, how was the victim looks like, any suspicious person lurking at the scene, and also give a brief summary of the incident. In the incident reports, you have to bring out the 5Ws and 1H things into the report to make it more informative and easier for the police to follow up and crack the case. The description of the accused if he fled the scene, the car plate number, the color of the car, what brand of the car the accused was driving on etc.

When filling out an incident report they want you to report who, what, when, where, why and how. Then they ask for a summary of the incident that occured. If it happened to the person reporting the incident at work they may require a drug test and a trip to urgent care depending on what is being reported.
