In order to obtain a death certificate you need to have a medical certificate from the doctor who was treating the person before they died. It is also a good idea to take the person's NHS medical card with you if you can find it.
If for whatever reason the coroner has had to be involved in the death then the office will also need some form of documentation from them. As well as documents you will also be asked to provide various information including the place of birth and the date of birth of the person who has died. They will also need to know the person's maiden name if they were married and the date of birth of the partner of the person who has now deceased. Basically they just need a little information on the person who has died and their partner if they have one.
